Many serious accidents have happened because of a machine that was running when it should have been turned off to allow for maintenance or repair. These incidents are typically caused by energy that has not been released properly. Lock-out-tag-out (LOTO) which makes sure that machines are off until a person with authorization releases them, helps prevent these kinds of accidents.

All "affected", "authorized", and "qualified" personnel must be instructed on the use of lockout and tagout devices. These devices include, but aren't restricted to chains, locks and padlocks. Each device is unique and labelled, including the date and time at which it was used along with the name and address of the person who is authorized, and the purpose of the lockout or tagout (repairs maintenance, repairs or repairs, etc.). It must also be clearly stated that these devices cannot be removed until a complete LOTO procedure has been carried out.
